Insurance Claims:
Our Patient Relations Coordinator will be happy to fill out your insurance forms at no cost to you. She will submit them for payment to your insurance company. This service will help you to maximize your dental benefits. However, our contract for dental treatment is with you, not your insurance company.

Payment Options:
Financial options are discussed during the initial visit. Dr. Noruzi and his staff are committed to providing excellent dental care and guide patients in choosing the best payment option for their individual needs. We accept cash, check, Visa, MasterCard, and Discover. Financing options through
Springstone Patient Financing is also available.

Payment is requested at the time of service, unless the patient has made other arrangements in advance. This policy reduces our billing costs and enables us to keep our fees as low as possible. You will always be given an itemized receipt upon completion of each dental treatment.

Appointment Scheduling Details:
At Fox River Periodontics we recognize that your time is valuable.  Consequently, we make every effort to maintain our appointment schedule to accommodate your needs.  If you are unable to keep your appointment, we ask that you notify our office a minimum of 48 hours prior to your scheduled appointment.
